Abdominal radical hysterectomy (ARH) is superior to radiochemotherapy (R-CT) in patients diagnosed with stage IB3 cervical cancer, according to the FIGO 2018 staging system, based on higher DFS and 5-year OS rates.

Cervical cancer is a prevalent malignant condition of the genital tract of the female. It is the leading cause of death across the globe, particularly in developing countries. Stage IB of cervical cancer is further categorized into stages IB1, IB2, and IB3 based on tumor size. This study, published in the journal Frontiers in Oncology, compared survival outcomes for radiochemotherapy (R-CT) to those for abdominal radical hysterectomy (ARH) in patients with stage IB3 cervical cancer.

The authors used propensity score matching (PSM) to compare the R-CT group and ARH group on the basis of disease-free survival (DFS) and 5-year overall survival (OS) rates. The study recruited a total of 590 patients diagnosed with stage IB3 cervical cancer using the FIGO 2018 staging system. The R-CT group comprised 120 patients, whereas the ARH group included 470 patients. In the total study sample, different DFS and 5-year OS rates were observed in ARH and R-CT groups. These DFS and 5-year OS rates were relatively higher in the ARH group as compared to the R-CT group. Through Cox multivariate analysis, the R-CT group was found to be associated with worse 5-year OS and DFS rates. Following 1:3 PSM, DFS and 5-year OS rates were relatively lower in the R-CT group.

In summary, the study demonstrated higher DFS and 5-year OS rates in patients who underwent ARH for stage IB3 cervical cancer. The study findings indicate that ARH is associated with better oncologic outcomes in patients with stage IB3 cervical cancer. The study findings are comparatively different from the NCCN guidelines and radiotherapy recommendations. More prospective clinical trials are required to confirm the appropriate treatment plan for individuals with IB3-stage cervical cancer, according to FIGO 2018.
